Consent Form for CIE MathComp/MathFun

My child _____________ has my permission to participate in the MathComp activities and MathFun games in the 2007 CIE MathComp event at CCCC, on May 26, 2007.

The undersigned as legal guardian of the student do hereby grant to the CIE officers and volunteers of the MathComp/MathFun event in my absence, the authority to render a judgment concerning medical assistance in the event of an accident. I do not hold the CIE/USA DFW chapter officers, volunteers responsible for any injuries occurred during the activities provided by CIE/USA DFW chapter, and I so signify by signature below.
